If you would like to register as a patient of doctor Loreta Graževičiūtė, the GK Clinic’s family doctor, please contact the registration desk where you will be able to fill in an Application form to register as the doctor’s patient.
Registration requires the following documents:
For adults: an identification document with a photo (passport, ID card or any other personal document with a photo and personal identification number).
For children up to 18 years of age: a Lithuanian birth certificate with a personal identification number.
Only individuals with compulsory health insurance are eligible for registration at the institution read more
The GK Clinic offers a 20% discount for other treatment services at the clinic for patients registered at the family centre.
CHANGING YOUR HEALTHCARE INSTITUTION
Medical records are transferred within 3 working days of the date the application is received. The patient does not need to fill in this application form, it is filled in by the healthcare institution itself.
STATE FUNDED TREATMENT IS AVAILABLE TO INDIVIDUALS WHO HAVE COMPULSORY HEALTH INSURANCE
The following healthcare services are funded by the Compulsory Health Insurance Fund:
– preventive medical aid
– medical aid, medical rehabilitation
– nursing, social services and other services associated with personal healthcare.
